Birth Records

Uncertified copies of birth certificates for genealogical purposes are available to individuals, who may not otherwise be entitled to receive a certified copy, upon completion of the Application for Search of Birth Record Files (see  FORMS in the right-hand column), if the person's date of birth precedes the current date by 75 years or more. Eligibility will be determined upon receipt and review of the documentation submitted. You can also contact the county clerk in the county where the birth occurred (see RESOURCES in the right-hand column and click on List of County Clerks) 

Death Records

Uncertified, genealogical copies of death records of persons who died more than 20 years ago, but not before 1916, may be requested either from the Illinois Department of Public Health, Division of Vital Records or from the county clerk in the county where the death occurred (see RESOURCES in the right-hand column and click on List of County Clerks).

Uncertified, genealogical copies of death records of persons filed before 1916 may be obtained from the county clerk in the county where the death occurred (List of County Clerks) or from Illinois Regional Archives Depository System if it holds death records for that particular county (see  RESOURCES in the right-hand column and click on List of County Clerks)
