EMS Licensing

IDPH licenses people in the State of Illinois to assure they are competent and keep current in the practice of Emergency Medical Services. This includes the licensing of EMT-Basic, EMT-Intermediate, Advanced EMT, Paramedic, Lead Instructor, Emergency Medical Dispatcher, First Responder, Pre-Hospital Registered Nurse, and Emergency Communications Registered Nurse. Much of the Illinois EMS personnel licensing process can be transacted on online, using the links and forms available on this page.

01/01/2023: For license renewals occurring on or after January 1, 2023, Emergency Medical Services personnel must complete at least one one-hour course of training on the diagnosis, treatment, and care of individuals with Alzheimer's disease or other dementias per license renewal period. This training shall include, but not be limited to, assessment and diagnosis, effective communication strategies, and management and care planning.

11/23/2021: In an effort to have the Illinois Department of Public Health’s licensing database congruent with the Administrative Code, the former license type “ A-EMT/ EMT-I – Advanced/ Intermediate Emergency Medical Technician” will be split out for each individual license types AEMT or EMT-I. The two new licenses types will be “A-EMT – Advanced Emergency Medical Technician” and “EMT-I –Emergency Medical Technician - Intermediate.”

As of September 20, 2018, individuals applying for Reciprocity must submit the $50 application fee as well as the amount of the initial license.  For individuals requesting out of state license verification, there is a $25 processing fee.
